Community Health & Wellness Grants
A broader focus on community health (population health) is changing when, where, and how health care is delivered. We prioritize transformational ideas that create access to care where it did not previously exist, and projects that demonstrate a clear and measurable impact on improving the social determinants of health in Ashland, Talent, and Phoenix.
